The differences in mean episode duration may reflect severity (no data were given for this). The people who recovered quickly on their own may have done so because they were less depressed. It stands to reason that those who sought help at the mental-health-system level were probably more depressed, hence took longer to recover. In any case, the point is that today, as in Kraepelin's time, many depressed patients recover, with or without medical intervention, because that's the nature of the illness. It comes and it goes."
